Working Together – Making a Difference
The benefit of this program is the individualized care and team approach, with all members focusing on their area of clinical expertise.

By working together, they can develop and implement a comprehensive plan of care that addresses the entire patient’s unique needs, resulting in optimal patient outcomes. We pride ourselves on our clinicians’ expertise and understanding of the most appropriate and effective wound care strategies supported by current research that enable our patients to regain function, strength, and ability to achieve the independence to safely return home.

Our teams coordinate the correct mix of physical and occupational therapies and modalities, along with physician and nursing care to meet each individual patient’s needs and provide recovery.

We work to develop goals with input from each resident to help achieve the greatest possible success.
