Parkway Pavilion Health & Rehabilitation Center is focused on simplifying the admission process to our facility and reducing unnecessary hospital admissions and readmissions.

When the time comes to return home:
- On-site case management and social services to assist with discharge planning and allocation of resources
- Home evaluations and safety assessments by our therapy team
- Medication management and education with return demonstration for residents and family to ensure safe administration
- Collaboration with home care agencies, senior resources and caregivers
- Scheduled follow-up visits with your doctor and detailed discharge summaries
